In the U.S., the majority of private and public payers use the Medicare Resource-Based Relative Value Scale (RBRVS) as the starting point for physician fee. Since pediatric care is under-represented in any Medicare-based payment system analysis, unique aspects of physician work and practice expense may not be correctly reflected in the total relative value units (RVUs) for certain pediatric services. In spite of this potential limitation, the American Academy of Pediatrics still uses the RBRVS physician fee schedule as a uniform payment system.
