In the Orthopedic specialty, we have a tendency to under code our established visits and over code our new patient visits. During this 60 minute audio, expert speaker Margie Scalley Vaught, CPC, CPC-H, CPC-I, CCS-P, PCE, MCS-P, ACS-EM, ACS-OR will walk you through the common errors and pitfalls when it comes to supporting documentation of the services being rendered. You will learn when it would be appropriate to use a modifier 25 vs. 57 and when no E/M should be reported. 

Margie will provide examples of what a 99203, 99214, 99213 and 99212 may look like from an E/M documentation guidelines setting. She’ll also provide information of targeted and audited areas by the OIG work plan along with the CERT auditors.

  • What is really a ROS vs. Past history and do you have at least 10 on your intake forms?
  • When can a comprehensive examination be supported and what would it look like?
  • There will be discussion as to how to help your providers, get the documentation in the medical records that they are actually doing and performing.
  • Issues of ‘cloning’ will be discussed and how to avoid when using EHR.
  • Other issues such as ‘timely documentation’, ‘not documented/not done’ and what about lost documentation or never done documentation.
