MPFS 2013 Update: The CMS has finalized new Medicare coverage for a couple of transitional care management (TCM) codes. These TCM codes are planned to pay a patient’s physician or practitioner to manage the patient’s care in the 30 days subsequent to a hospital or nursing facility stay.
