MPFS 2013 Update: The CMS has finalized new Medicare coverage for a couple of transitional care management (TCM) codes. These TCM codes are planned to pay a patient’s physician or practitioner to manage the patient’s care in the 30 days subsequent to a hospital or nursing facility stay.

The final 2013 MPFS rule involves a lot of changes when it comes to PQRS, eRX, Physician Value-Based Payment Modifiers, and much more.
