Imagine a CMS surveyor who shows up at the hospital to do a CMS validation survey. The surveyor pulls out the final infection control worksheet which has a section on safe injection practices. Now compliance with safe injection practices will be assessed during all validation surveys, certification surveys, and resurveys. Hospitals have reported surveyors showing up at their doorstep just to go over the three worksheets.
This is especially important in light of a number of cases where patients have died or suffered an adverse event in the past year. CMS has issued many important survey memos related to safe injection practices and infection control. In a memo issued by CMS on this subject, it talked about what are expected by hospitals to do, to be compliant with CDC standards on safe injection practices. CMS issued another survey memo on four unsafe injection practices that can end up with a second knock at the hospital’s door from the state epidemiologist. Many hospitals are currently out of compliance!
Additionally, all hospitals should be in compliance with the 10 requirements of the CDC safe injection practices. Do you know what these ten requirements are and how do you ensure you are in compliance? Does your ED physician wear a mask when doing a LP or your anesthesiologist when they insert an epidural for pain relief? If they make it in a single dose then the hospital needs to purchase it. Do you know what the rules are now for multi-dose vials? Does your staff know these cannot be taken into patient rooms including the OR with exception? Every hospital should have a policy on this. All staff, including physicians, should be trained on this. Staff needs to be educated on the ten CDC standards for safe injection practices and assessment with compliance should be done.
CMS also requires hospitals to follow standards of care such as those issued by the Institute for Safe Medication Practices (ISMP). The policy should be amended to include these standards in addition to education of the staff. Injected medicines are common in healthcare today. Injection safety or safe injection practices are measures taken to perform injections in an optimally safe manner for patients. Many hospitals and other facilities have just taken for granted that, this was being performed appropriately but recent studies show this to be untrue. A physician in Nevada was recently sent to prison for being out of compliance with these standards. State boards have taken away the licenses of nurses and physicians and pharmacists for violating safe injection practices.
Every hospital that accepts Medicare or Medicaid reimbursement should listen to this important program!
