Continuous home care is a challenging level of care for a hospice to provide effectively and is one of four levels of care that must be provided by hospices under the Medicare Hospice Benefit.
This session by expert speaker Charlene Ross MSN, MBA, RN, will review the regulations related to continuous home care level of care and help hospices to identify the patients appropriate for continuous home care. It will also explore elements a hospice should have in place for a successful, cost effective and compliant continuous home care program.
There are grey areas in the eligibility requirements which make it a challenge to properly identify and apply good assessment and documentation practices. In order to help ensure proper utilization, hospices must understand the practical application and the regulatory requirements. This session will help hospices understand the black and white, and learn to accept the grey areas of regulatory requirements for the continuous home care level of care under the Medicare Hospice benefit. In addition, the session will provide some best practices on how to be successful.
